Mapping Controls to Risks

The most common structural weakness in ISO 27001 implementations is not a missing control or an undocumented policy — it is the absence of a coherent thread connecting the risk register to the controls that treat those risks to the evidence that proves the controls are working. Without this thread, the ISMS is a collection of individual documents rather than a management system. With it, every component becomes traceable to every other component — and the ISMS can be navigated, interrogated, and defended from any direction.

A controls-to-risk traceability matrix is the artifact that creates this thread. It is not a new ISO 27001 requirement — the standard requires risk treatment (Clause 6.1.3), a Statement of Applicability (Clause 6.1.3), and documented risk treatment results (Clause 8.3) but does not mandate a specific matrix format. The traceability matrix is the practical implementation that satisfies all three requirements simultaneously while also answering the questions that auditors, management, and regulators actually ask.

This article covers why traceability matters and what it enables, the matrix structure and what each column contributes, a worked example covering five real-world risks with full control mapping, the four directions the matrix must be navigable in, the eight-step process for building it, and the most common ways organizations get it wrong.

Why Traceability Matters: Five Use Cases

A well-constructed traceability matrix serves five distinct purposes simultaneously — each answering a different question from a different stakeholder. Understanding these purposes helps define what the matrix needs to contain and how it should be structured:

THE CONNECTING THREADThe traceability matrix creates the connecting thread between four ISMS artifacts that are often built in isolation: the risk register, the SoA, the risk treatment plan, and the evidence library. Organizations that build these four artifacts without connecting them produce an ISMS that looks complete on paper but cannot be navigated during an audit. An auditor who pulls one thread — 'show me how you treat your highest-rated risk' — should be able to follow it all the way to specific evidence in under two minutes. The matrix makes this possible.

Matrix Structure: What Each Column Does

The traceability matrix has 11 columns, drawn from four source documents: the risk register, the SoA, the risk treatment plan, and the evidence library. Each column has a specific purpose — understanding the purpose of each column prevents the common mistake of building a matrix that looks complete but cannot answer the questions auditors actually ask:

Column priority: Not all 11 columns are equally critical for the initial build. The Four Directions of Traceability

A genuinely useful traceability matrix is navigable in four directions — not just one. Organizations that build a matrix only navigable in one direction (e.g. control → risks) cannot answer questions posed from a different direction (e.g. risk → controls). Auditors and regulators ask questions from all four directions:

In practice, a well-structured spreadsheet or GRC platform makes all four directions available through filtering, sorting, and lookup. A risk-centric matrix (rows organized by risk) is naturally Risk → Control. A control-centric matrix (rows organized by control) is naturally Control → Risk. The most flexible format uses a risk-centric primary view with control reference cross-indexes that support lookup from the control direction.

Worked Example: Five-Risk Traceability Matrix

The worked example below covers five representative risks from a financial technology organization operating in Indonesia — showing the complete traceability from risk ID through control selection, implementation status, residual risk, ownership, and regulatory links. This is the format that satisfies Stage 2 auditors and OJK examiners simultaneously:

Several features of this worked example are worth noting specifically. Risk R-003 (data exfiltration) shows a 'Partially' accepted status because one of the controls (8.12 DLP) is not fully deployed — honest partial acceptance is the correct response to a partially treated risk. Risk R-012 (supply chain attack) shows CRITICAL controls in partial status with active treatment plan items — this is what a mature risk treatment plan looks like in practice. Risk R-019 (laptop data breach) shows a LOW residual score because full disk encryption reduces the impact of a lost device to near-zero, demonstrating effective risk reduction through a focused technical control.

Building the Matrix: Eight Steps

The matrix is built in eight sequential steps — each building on the previous. Steps 1–3 establish the risk-control connections. Steps 4–6 add the assessment and acceptance layer. Steps 7–8 add the compliance and evidence layer that transforms it from an audit tool into a management system artifact:

Action: Export all risks rated HIGH (12+/25) or CRITICAL (20+/25) from the risk register. These are your priority rows. Lower-risk items can be added progressively but the HIGH/CRITICAL risks must all be in the matrix before Stage 1.

02Map controls to each risk

For each risk, identify which Annex A controls directly treat it. Use the ISO 27002 'Purpose' statement for each control to guide the mapping — the purpose statement explains what risk the control is designed to address.

Action: For each risk row: review the control statements and purpose descriptions for each Annex A domain. Map the controls that directly reduce the likelihood or impact of the specific risk. Be specific: 'A.8.5 reduces the likelihood of unauthorized access to payment accounts by requiring MFA' is a valid mapping. 'A.5.1 policies generally support security' is not.

03Verify SoA consistency

For every control appearing in the matrix, it must be listed as 'Applicable' in the SoA. If a control appears in the traceability matrix but is excluded in the SoA, there is an internal inconsistency that will generate a Stage 1 finding.

Action: Cross-check every control in the matrix against the SoA. If a control treats a HIGH/CRITICAL risk but is excluded from the SoA, the exclusion justification must be reconsidered — it is very difficult to justify excluding a control that directly treats a significant risk.

04Record implementation status per control

For each control mapped to a risk, record its current implementation status: Implemented / Partial / Planned / Not started. Status must reflect reality at the time of the matrix update.

Action: Use four status categories and their specific meanings: Implemented (evidence exists and is current), Partial (control exists but coverage is incomplete — e.g. MFA deployed for 80% of accounts), Planned (formally in the risk treatment plan with a target date and owner), Not started (identified in RTP but no action taken). 'Partial' must include a note on what is missing.

05Calculate residual risk scores

For each risk, calculate the residual risk score — the score after controls are applied. Residual score is not mechanical (score minus a fixed reduction) — it requires judgment about how much each control actually reduces likelihood and impact given its current implementation status.

Action: Apply the same L×I methodology as the risk assessment. For each risk: given the controls now in place and their implementation status, what is the realistic likelihood and impact if the threat now materializes? Document the reasoning. A control that is 'Partial' should not be given full credit in the residual calculation.

06Assign risk owners and record acceptance

For each risk, identify the risk owner — the business manager responsible for the assets at risk — and record their formal acceptance of the residual risk. Acceptance must be documented, named, and dated.

Action: Risk owners must be business managers with authority over the affected assets — not the ISMS Manager or IT team. Each risk owner receives a formal risk briefing showing the inherent risk, the controls in place, and the residual risk they are being asked to accept. Their acceptance is documented in the matrix and the risk register.

07Add regulatory links

For each risk-control combination, identify the Indonesian regulatory requirements that are addressed. This column transforms the traceability matrix from an ISO 27001 tool into a multi-framework compliance artifact.

Action: For each row, identify: which UU PDP articles are relevant (data protection, breach notification, data subject rights), which POJK requirements apply (IT governance, operational risk, outsourcing), and whether BI payment system standards are relevant. Reference specific article numbers, not just regulation names.

08Add evidence pointers

For each 'Implemented' or 'Partial' control in the matrix, add a pointer to where the evidence lives. Evidence pointers should be specific enough that the ISMS Manager can retrieve the evidence during an audit within 30 seconds.

Action: Use a consistent format: [Library location] / [Document name] / [Date] for documents. [System name]: [specific record] for system evidence. If using Bitlion GRC: record the control ID in the platform which links directly to the evidence record.

Build timing: The traceability matrix should be initiated in Phase 2 (Risk Assessment) and progressively populated through Phase 4 (Control Implementation). It is NOT a document to be built in the final weeks before Stage 1. A matrix built in a rush before Stage 1 will be generic, inconsistent, and unconvincing. A matrix built progressively over the implementation period will be specific, evidenced, and defensible. Start with the HIGH/CRITICAL risks in Phase 2 and add lower risks progressively as implementation matures.

Using the Matrix at Management Review

The traceability matrix is not only an audit artifact — it is the primary data source for management review. Four of the eight required Clause 9.3.2 inputs can be derived directly from the matrix. Using the matrix at management review demonstrates that governance is data-driven rather than intuition-driven:

The common thread across these failures is the same pattern seen throughout the documentation series: the matrix was built for the audit rather than for the management system. A matrix built to satisfy a Stage 1 package requirement will be generic, static, and disconnected from operational reality. A matrix built as a genuine risk management tool will naturally satisfy the audit because it reflects how the organization actually tracks and treats its information security risks.

The highest-value use of a well-maintained traceability matrix is not audit preparation — it is organizational decision-making. When the CISO needs to make the case for a new security investment, the matrix shows which high-residual risks are currently under-controlled and which specific controls would reduce them. When the board asks whether the organization's security spend is addressing the right risks, the matrix shows the connection between expenditure (controls implemented) and risk reduction (residual score movement). When a new regulation introduces new requirements, the matrix shows which existing controls already address the new obligations and where genuine gaps exist.

This is what distinguishes a mature ISMS from a compliance exercise. The compliance exercise produces documents that satisfy an auditor. The mature ISMS produces documents that the organization uses to govern itself — and that satisfy auditors as a byproduct of being genuinely well-managed. The traceability matrix, maintained as a living management tool rather than a static compliance artifact, is one of the clearest signals of which kind of ISMS an organization has built.
